ADOPTION QUESTIONNAIRE:

Thanks for your interest in a Chinese Crested puppy from Punjin Chinese Cresteds. We are interested in placing the right puppy in the right home. So please copy and paste this questionnaire into and email to me. We strive to produce healthy and happy Chinese Cresteds for show and pet homes

1. Where will your new puppy live and where will the puppy live when you are away from home.
   
2. Do you have a fenced yard? If not how will the puppy get exercise and potty.
   
3. Do you do regular vet checks and yearly heartworm preventative? Please add the name of your Vet and phone number. You must give permission for your Vet to speak with me.
   
4. Do you have any other pets? If so what breed and ages? Are your other pets spayed and neutered.
   
5. Do you have children and what ages are they. Are you married or single and is your spouse agreeable to a new puppy?
   
6. Do you live in an apartment or a house? Do you own or rent? If you rent please add the name and number of your landlord. Are you willing to have a home check or send pictures of your yard.
   
7. How much time are you away from home and what is your occupation?
   
8. If you are looking for a Hairless dog do you know they do come in a variety of body hair are you willing to keep the puppy that requires some grooming as the Hairless can be as much work as a powderpuff.
   
9. Are you looking for a hairless or a powderpuff and a girl or a boy?
